How to use the e-portal form Application form

Answer:

a. Take a photo of your signature signed into a sheet of paper, snap it and add it as a picture in jpg, png or jpeg format.

Answer:

Click on the date field and a calendar will pop up to choose the date where applicable.

Click on choose a file field, select the passport from your phone or laptop and add as specified.

Fill in the first field/row if it is only for an individual or add more individuals details in the fields specified below according to the numbers of individuals you are applying for.

Treatment and claims form

Answer:

a. Fill in the laboratory/radiological investigation and the amount for just an individual field/row or fill more than the rows if we have more laboratory/radiological fields and amount.

Click on the date field and a calendar will pop up to choose the date where applicable.
Fill in the dates of birth with the right data type, a calendar will pop. For age, use only numbers.
Please fill all fields where applicable else the form won’t be submitted.
